Employees are tasked with servicing and maintaining equipment including but not limited to presses, cutoff equipment, coilers and take up reels. Hazardous energies present on equipment includes hydraulic, pneumatic, gravitational, electrical, and mechanical.

Hazardous energy sources were not located and operated in a manner to isolate the hazardous energy sources, the equipment was not tested to verify isolation, and lockout devices not affixed to the isolation points.

Fixing of selection was not by means capable of supervision by the employer; On or about July 14, 2020 the employer failed to ensure that the inch. single stroke, and continuous selection switch was under supervised control. Operators routinely have access to and operate the selection switch as needed.

29 CFR 1910.217(c)(3)(iii)(c): Presence sensing point of operation device(s) on mechanical power press(es) were not constructed so that a failure within the system does not prevent the normal stopping action from being applied to the press when required, but does prevent the initiation of a successive stroke until the failure is corrected. The failure shall be indicated by the system. On or about July 14, 2020 the employer failed to ensure that the failure of the presence sensing device on the Aida 300 ton press prevented successive strokes from occurring. The light curtains on the press were not functioning, however the press was able to operate in continuous mode following the clearing of a presence sensing device without actuation of the two hand control devices.
