OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: On or about July 15, 2020 at a job site located at 1600 Pennsylvania Ave, Salem, Ohio 44460; an employee was exposed to a fall hazard of approximately 7 to 12 feet while using the bucket of a Case Extendahoe 580 Super L, as a means of access and or egress to replace a hydraulic line on an excavator.
