29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns. At the jobsite, employees were not wearing hardhats while working below other employees that were installing cornice trim to a residential house, exposing employees to struck-by hazards from falling objects.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents. At the jobsite, employees operating a Metabo HPT nail guns while installing cornice material were not wearing eye protection, exposing employees to the hazard of being struck by flying objects.

29 CFR 1926.405(g)(2)(iii): Splices. Flexible cords shall be used only in continuous lengths without splice or tap. At the jobsite, employees used a yellow extension cord that was spliced to a black power cord and wrapped with black tape, supplying power to a Makita Skill saw, and exposing employees to the hazard of electrical burns and shock.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level. At the jobsite, employees working from the wooden scaffold were not provided fall protection or a guardrail system exposing the employees to the hazard of falling more than 15ft. to the ground below.
