29 CFR 1910.178(l)(1): The employer did not ensure that each power industrial truck operator was competent to operate a power industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in paragraph (1):    At jobsite 1715 N. 8th Neodesha KS 66757, employees engaged in plastering activities were exposed to serious crushing and struck-by injuries. Forklift operator had not completed a forklift training program that met the requirements of OSHA standard 1910.178(l) powered industrial trucks, operator training.
