29 CFR 1926.501(b)(15): Each employee on a walking/working surface 6 feet (1.8 meters) or more above lower levels was not protected from falling by a guardrail system, safety net system, or personal fall arrest system: (a) Adventure Development Team, LLC located at 28194 US Hwy 6 Dillon, CO 80435: On or around 7/18/2020 the employer did not ensure each employee on a walking/working surface having an unprotected side or edge which was six or more feet (1.8m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest system. Employee(s) were constructing a new aerial ropes course. An employee was not protected from a fall while evaluating ropes course for need materials. The employer did not ensure that all employees were protected by guardrail systems, safety net systems, or personal fall arrest systems. This condition exposed employee(s) to approximate 17 foot fall hazards, and an employee sustained injuries.

Employee(s) were constructing a new aerial ropes course. An employee was not protected from a fall while evaluating ropes course for need materials. The employer had not provided employees fall protection training to recognize the hazard and steps to mitigate it. This condition exposed employee(s) to approximate 17 foot fall hazards.
