In the Gallery of the Argonia Elevator,  the employer did not maintain the hallway floor leading into the annex in a safe condition in that the plate that covers the gap in between the galley and annex was bent up approximately 4 inches off the ground, exposing employees to a slip, trip, and fall hazard.

29 CFR 1910.272(k): Receiving pit feed openings in the grain handling facility, such as truck or railcar receiving pits, were not covered by grates which had a maximum opening width of 2 1/2 inches:  At the truck unloading pit, grate number one contained openings in the grate that measured approximately 3.5 to 4 inches, exposing employees to a possible explosion hazard due to potential tramp metal and foreign objects being introduced into the grain conveyance system.
