29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles: On or about 07/20/2020, during the installation of roofing material at the job site located at 810 Windy Hill Road, Smyrna, GA 30080, the employer failed to ensure that employees working on the roof of an apartment complex were protected from possible eye injuries, from impact or flying objects from other employees installing roofing material. Two employees were using pneumatic nail guns to secure roofing materials to structure without using safety or facial protection.

29 CFR 1926.300(b)(2): Moving part(s) of equipment exposed to contact by employees or which otherwise created a hazard, were not guarded: On or before, and at times prior to 07/20/2020; adequate guarding was not provided for the vertical incline belt and starter recoil of the ROL-AIR Compressor. Employees who used the compressor as a power source for pneumatic tools were exposed to caught-by, struck-by hazards from unguarded rotating parts.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: On 07/20/2020, employees tasked with tearing off and installing asphalt roofing materials on a residential property located at the Carriage House Apartments, 810 Windy Hill Rd. (Apt. #15) were not protected from a fall hazard by conventional fall protection and/or devices. Employees were exposed to fall hazard from a height of approximately 18 feet from the eave height to ground level.
