Uncertified operators were in close proximity to others working on the roof and at ground level.

(Apt. #05) were not protected from a fall hazard by conventional fall protection and/or devices. Employees were exposed to fall hazard from a height of approximately 16-feet, 3-inches from eave height to ground level.
