29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: (a) Facility: On or about July 16, 2020, there were two authorized employees working on the standing seam roof machine (SSR) and the machine was not locked out nor tagged out by either employee. The employees had removed 4 guards and were working past where the guards were to do their work.

(a) Facility: On or about July 16, 2020, the two authorized employees did not ensure that all employees were removed from the work area before jogging the machine (to help set it up).

29 CFR 1910.147(c)(7)(iii)(B): Additional retraining was not provided whenever a periodic inspection under 29 CFR 1910.147(c)(6) revealed that there were deviations from or inadequacies in the employee's knowledge or use of the energy control procedures: (a) Facility: On or about July 16, 2020, two authorized employees were working on a machine (standing seam metal roof) and it was not locked nor tagged out. These two authorized employees had removed 4 panel guards to the machine and were working inside the area of the removed guards. These two authorized employees need to be retrained in the proper procedures of lockout/tagout.
