On or about July 23, 2020, in the facility, employees were exposed to hazards including, but not limited to electrical and laceration hazards when the energy control procedure for Combi Model 2EZHS Ergopack Serial Number 2EZHS588660 on Line 19 omitted a source of energy. The omission of the energy source exposed employees to unexpected start up of the machine.

On or about July 11, 2020, employees in the packaging area, were exposed to amputation and laceration hazards when not provided with training to recognize applicable hazardous energy sources and methods and means for energy isolation when performing tasks including, but not limited to, unjamming equipment. b. On or about October 31, 2020, employees in the packaging area, were exposed to amputation and lacerations hazards when not provided with training to recognize applicable hazardous energy sources and methods and means for energy isolation when performing tasks including, but not limited to, unjamming equipment. Vee Pak, LLC dba Voyant Beauty was previously cited for a violation of an equivalent Occupational Safety and Health Standard 29 CFR 1910.147(c)(7)(i), which was contained in OSHA inspection number 1415105, citation number 1, item number 3, and was affirmed as a final order on November 14, 2019, with respect to a workplace located at 5331 Dansher Road, Countryside, IL 60525.
