29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: a) Jobsite: 4119 Butler Street, Pittsburgh, PA 15201, on or about July 23, 2020, employees were not protected from falling while working on a ladder jack scaffold in order to repair mortar on the side of a brick building. The ladder jack scaffold was set up on a flat roof of the adjacent building and employees were exposed to a fall hazard of approximately 30 feet to the concrete sidewalk below.

29 CFR 1926.454(c)(3): The employer failed to retrain employees that showed indications through work performance of not retaining the requisite proficiency: a) Jobsite: 4119 Butler Street, Pittsburgh, PA 15201, on or about July 23, 2020, employees that were working on scaffold did not have adequate training in the use of fall protection when working on the scaffold.
