The employees were not trained to recognize, eliminate and avoid unsafe conditions as outlined in 29 CFR 1926, Subpart P. The employees were working in a trench measuring 28 feet deep X 15 feet wide X 39 feet long.

29 CFR 1926.652(a)(1): Each employee in an excavation shall be protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: On and prior to 2/7/2020, employees of Aarrow Boring, Inc. were operating a boring machine at a jobsite located at 11660 Administration Drive, Saint Louis, Missouri. The employees were exposed to engulfment and struck-by hazards while performing work in a trench measuring 28 feet deep X 15 feet wide X 39 feet long. The employer did not ensure protective system being used inside the trench was adequate to protect employees.
