29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: Location: 24 Essex Street, Haverhill, MA, Basement On or about 07/22/2020, employees were not adequately instructed in the recognition and avoidance in unsafe conditions while attempting to lift an overhead HVAC unit that slipped off of its drainage tray, injuring an employee's arm.
