29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  (a) On or about July 28, 2020, employees conducting ground work activities below overhead siding work, did not wear head protection, exposing employee(s) to struck-by hazards from material and tools from the upper level roof area, where work was being performed.

29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee use appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors or potentially injurious light radiation.  (a) On or about July 28, 2020, employees did not use eye protection when using pneumatic nail gun, exposing employees to eye foreign body/impalement hazard from nails.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  (a) On or about July 28, 2020, employees conducting siding work on a 4:12 pitch roof were not protected from falling to the lower level. This exposed employees to a fall hazard of approximately eight (8) feet ten (10) inches while working on the North and South sides of the structure.
