29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to the environment to control or eliminate any hazards or other exposure to illness or injury: a) Resource Center parking lot: On or about July 29, 2020 and at times prior thereto employees worked in a 6 foot deep trench without a cave-in protective system or safe means of egress in place. The employees did not recognize the hazard of working unprotected in a trench without a safe means of egress, or the actions needed to control the hazards.
