29 CFR 1926.651(j)(2): Employees were not protected from excavated or other materials or equipment that could pose a hazard by falling or rolling into excavations, or by a combination of both if necessary. On or about July 28, 2020, employees were exposed to a struck by hazard while performing sewer line replacement at a residential home located at 1945 W Sprague, Broadview Heights, Ohio 44147. The employees were working in a trench and the employer did not ensure that tools and spoil piles were set back at least two feet from the edge.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652. On or about July 28, 2020, employees were exposed to a trench cave in while performing sewer line replacement at a residential home located at 1945 W Sprague, Broadview Heights, Ohio 44147. The employees were working in a trench and the employer did not ensure that they were protected from a cave in.
