29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):  (a) Worksite:  On or about 7/30/2020, employees were performing work on a roof level, approximately 14 feet in height, without using fall protection.  Dennis Robinson Roofing, Inc. was previously cited for a violation of this occupational health and safety standard which was contained in OSHA inspection #1251255, citation1, Item 1a and was affirmed as a final order on 10/17/2017, with respect to the workplace at 451 Pawtucket Ave., Pawtucket, RI 02860.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer:  (a)  Worksite:   On or about 7/30/2020, the employer did not have a written record of fall protection training for employees who were exposed to fall hazards of approximately 14 feet in height.   Dennis Robinson Roofing, Inc. was previously cited for a violation of this occupational health and safety standard which was contained in OSHA inspection #1251255, citation1, Item 1b and was affirmed as a final order on 10/17/2017, with respect to the workplace at 451 Pawtucket Ave., Pawtucket, RI 02860.
