29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about July 30, 2020, where employees were installing a new roof on a residential home located at 536 Greenside Drive, Painesville, OH 44007, the employer did not ensure that employees were protected from falling a distance of greater than 14.8 feet to the ground below. Angel Colon Salgado was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1320079, citation number 1, item number 3, and was affirmed as a final order on August 3, 2018, with respect to a workplace located at 9943 Chamberlin Road Twinsburg, Ohio 44087.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about July 30, 2020, the employer did not ensure that a ladder utilized to access the roof of a residential home located at 536 Greenside Drive, Painesville, OH 44007 was extended at least 3 feet above the upper landing surface, exposing employees to a fall of approximately 10.5 feet. Angel Colon Salgado was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.1053(b)(1), which was contained in OSHA inspection number 1320079, citation number 1, item number 4, and was affirmed as a final order on August 3, 2018, with respect to a workplace located at 9943 Chamberlin Road Twinsburg, Ohio 44087.
