29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:     On or about July 30, 2020, when installing a new roof on a residential home located at 30425 Vineyard Road, Willowick, OH 44095, the employer did not ensure that employees utilizing air powered nail guns wore eye protection.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    On or about July 30, 2020, the employer did not ensure that employees performing work on the roof of a residential home located at 30425 Vineyard Road, Willowick, OH 44095 were protected from falling approximately 10.1 feet to the ground below.
