Summit's Trace Healthcare Center located at 935 North Cassady Avenue, Columbus, Ohio 43219: On or about July 31, 2020, the employer did not develop and implement a written respiratory protection program with worksite-specific procedures for respirator use that included all provisions in 29 CFR 1910.134(c)(1)(i) - (ix). The employer required employees to wear respirators throughout their shifts while exposed to suspected or confirmed positive COVID-19 residents.
