29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  Jobsite:  The employees who were installing asphalt shingles on the 8 in 12 high pitch residential roof were not protected from fall hazards approximately 11 feet 7 inches to the lower level by the use guardrails, safety nets or personal fall arrest systems.

29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person.   Jobsite: The anchorages used for attachment of the personal fall arrest systems were not capable of supporting at least 5,000 pounds for each employee attached, exposing employees to fall hazards approximately 11 feet 7 inches to the lower level.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:  Jobsite:  The vertical lifelines were not rigged in a manner that could prevent the employees from free falling approximately 11 feet 7 inches to the lower level while reroofing the 8 in 12 high pitch roof.
