On or about July 30, 2020, at the above addressed jobsite, an employee performing masonry operations from a tubular welded frame scaffold, approximately thirty-two (32) feet above the ground, was not protected by guardrails and end rails, thereby exposing the employees to fall hazards. b. On or about July 30, 2020, at the above addressed jobsite, an employee performing masonry operations from a tubular welded frame scaffold, approximately twenty-three (23) feet above a lower landing surface, i.e., scaffolding section, were not protected by end rails, thereby exposing the employees to fall hazards.

On or about July 30, 2020, at the above addressed jobsite, employees working below masonry activities were not wearing hard hats to protect against falling materials and debris. BM International Builders, Inc. was previously cited for a violation of this occupational safety and health standard, 29 CFR 1926.100(a), which was contained in OSHA inspection number 1390020, citation number 1, item number 1, and was affirmed as a final order on June 27, 2019, with respect to a workplace located at 504 N. Hermitage Ave. in Chicago, IL.
