On or about July 30, 2020, at the above addressed jobsite, employees were not trained in the recognition of jobsite hazards, which included, but were not limited to, the hazards associated with residential construction.

29 CFR 1926.102(a)(1): Employees did not use eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  a.  On or about July 30, 2020, at the above addressed jobsite, employees using pneumatic nail guns did not wear safety glasses, thereby exposing the employees to hazards associated with eye injuries.

On or about July 30, 2020, at the above addressed jobsite, employees performing residential roofing operations, nineteen (19) feet six (6) inches above the ground, were not protected from falls, thereby exposing employees to fall hazards.  b.  On or about July 30, 2020, at the above addressed jobsite, employees performing residential roofing operations, twenty-three (23) feet eight (8) inches above the ground, were not protected from falls, thereby exposing employees to fall hazards.

On or about July 30, 2020, at the above addressed jobsite, employees had been provided fall protection training, however, the employer failed to keep a written certification of record for each employee.
