29 CFR  1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:  a) Roof, New Cold Storage Building - On or about July 23, 2020 and at times prior to, employees working on the roof while installing metal decking on top of the roof insulation were exposed to fall hazards of up to approximately 27 feet, 7 inches in that the employees were not fall protected.

29 CFR  1904.39(a)(2): The employer failed to report the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, within (24) twenty-four hours:   a) At Facility - On or about July 23, 2020, the employer did not report an in-patient hospitalization within 24 hours of an employee who was hospitalized after falling from a roof and sustaining serious injuries.
