The employer did not provide employees, such as, but not limited to, Certified Nursing Assistants, Licensed Practical Nurses, Registered Nurses, and Housekeepers with a medical evaluation prior to the use of the respirators. ABATEMENT DOCUMENTATION AND CERTIFICATION REQUIRED FOR THIS ITEM
