29 CFR 1926.453(b)(2)(iv): Extensible and articulating boom platforms; employees shall always stand firmly on the floor of the basket, and shall not sit or climb on the edge of the basket or use planks, ladders, or other devices for a work position. On July 29, 2020 at 550 Village Park Drive in Plover, Wisconsin, an employee was climbing on basket guardrails of a Skyjack SJ66T boom lift; extending work reach during exterior door installation.

29 CFR 1926.503(a)(2)(iii): The employer shall assure that each employee has been trained, as necessary, by a competent person qualified in the use and operation of guardrail systems, personal fall arrest systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used. On July 29, 2020 at 550 Village Park Drive in Plover, Wisconsin, the employer did not provide required fall protection safety training for employees who were exposed to fall from elevation hazards while working at heights greater than six feet.
