29 CFR  1910.22(d)(2): Hazardous conditions on walking-working surfaces are corrected or repaired before an employee uses the walking-working surface again.  If the correction or repair cannot be made immediately, the hazard must be guarded to prevent employees from using the walking-working surface until the hazard is corrected or repaired; and   On or about August 10, 2020, employees were exposed to caught in hazards from potato conveyor  not having a standard guardrail system.

29 CFR  1910.215(a)(4):Work rests. On offhand grinding machines, work rests shall be used to support the work. They shall be of rigid construction and designed to be adjustable to compensate for wheel wear. Work rests shall be kept adjusted closely to the wheel with a maximum opening of one-eighth inch to prevent the work from being jammed between the wheel and the rest, which may cause wheel breakage. The work rest shall be securely clamped after each adjustment. The adjustment shall not be made with the wheel in motion.  On or about August 10, 2020, a bench grinder located in the fabrication shop was observed with tool rest opening to the wheel periphery opening of approximately one inch and one eighth.  This condition exposed employees to struck by hazards.

29 CFR  1910.215(b)(9): Exposure adjustment. Safety guards of the types described in Subparagraphs (3) and (4) of this paragraph, where the operator stands in front of the opening, shall be constructed so that the peripheral protecting member can be adjusted to the constantly decreasing diameter of the wheel. The maximum angular exposure above the horizontal plane of the wheel spindle as specified in paragraphs (b)(3) and (4) of this section shall never be exceeded, and the distance between the wheel periphery and the adjustable tongue or the end of the peripheral member at the top shall never exceed one-fourth inch.    On or about August 10, 2020, a bench grinder located in the fabrication shop was observed with tongue guard opening to the wheel periphery opening of approximately one inch and a quarter.  This condition exposed employees to struck by hazards.

29 CFR  1910.243(c)(1):General requirements. Abrasive wheels shall be used only on machine provided with safety guards as defined in paragraph (c) (1) through (4) of this section.  On or about August 10, 2020, portable grinders were observed being used at the fabrication shop that the wheel guards were removed. This condition exposed employees to struck-by hazards.

On or about August 10, 2020, the electrical charging cables of two battery units were observed with the insulation pulled leaving exposed copper wire near the strain relief connectors. This condition exposed employees to electrical shock hazards.

29 CFR  1910.305(g)(2)(iii):Flexible cords and cables shall be connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints or terminal screws.  On or about August 10, 2020, the electrical cables of two battery charging units were observed with the insulation pulled where electrical cables were not secure to their strain relief device that prevents pulling directly from the terminals where the cables or wires are connected.    This condition exposed employees to electrical shock hazards.
