29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    On or about August 3, 2020 the employer failed to ensure that machine specific procedures were in place for equipment and machines within the facility which employees were tasked with servicing and maintaining. Employees perform these tasks on equipment including but not limited to plastic extruders, straighteners, packaging equipment. Hazardous energies present on equipment includes but is not limited to mechanical, electrical, thermal, and pneumatic.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed;  On or about August 3, 2020 the employer failed to ensure that an annual inspection of LO/TO procedures for equipment had taken place at the facility. Employees are tasked with servicing and maintaining equipment in the facility including but not limited to extrusion equipment, packaging equipment, drill presses, compressors, and conveyors. Hazardous energies include but are not limited to pneumatic, mechanical, thermal, and electrical.

29 CFR 1910.147(c)(7)(i)(A): Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control.      On or about July 13, 2020 the employer failed to ensure that operators of the extrusion lines were trained as authorized employees for the control of hazardous energies and LO/TO program. Operators of the extrusion lines are tasked with and responsible for changeovers and setups on the equipment without isolating the hazardous energy sources. Hazardous energies present on the equipment include but are not limited to pneumatic, mechanical, thermal, and electrical.

29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s):    On or about July 13, 2020 the employer failed to ensure that LO/TO was performed on the line 5 extrusion production line when changeover activities were taking place. Employees do not isolate the energy sources for change over and setup tasks. The employees rely on turning off the equipment with typical power switches. Hazardous energy sources on the equipment include but are not limited to pneumatic, thermal, mechanical, and electrical.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    On or about July 14, 2020 the employer failed to ensure that effective machine guarding was on the inlet side of the Youngstown Plastic Tooling and Machinery take off machine (serial number 6-1933) located on line 5. Employees are able to reach the ingoing nip point of the rotating feeder line. The machine is equipped with a bump stop and relay mechanism, that when tripped immediately shuts the equipment down. This bump stop does not prohibit the employee from reaching into the hazard area.
