Lutz VA Medical Center located at 1500 Weiss Street, Saginaw, Michigan 48602: On or about August 4, 2020 and at times thereafter, the employer did not ensure healthcare employees performing in-person COVID-19 screening of patients, vendors, visitors and staff (all assessment for triage) at building entrances used personal protective equipment, such as but not limited to goggles or face shields, in order to protect employees from the hazard of contracting SARS-CoV-2, the virus that causes COVID-19.

29 CFR 1910.132(d)(2) as required by 1960.8(b): The employer did not verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated, the person certifying that the evaluation has been performed, the date(s) of the hazard assessment, and which identifies the document as a certification of hazard assessment: Aleda E. Lutz VA Medical Center located at 1500 Weiss Street, Saginaw, Michigan 48602: On or about August 4, 2020 and at times thereafter, the employer had not verified, through a written certification, that the required workplace hazard assessment had been performed to determine if hazards such as SARS-CoV-2, the virus that causes COVID-19, were present that necessitated the use of PPE, such as goggles or face shields, in order to protect healthcare employees from the hazard of contracting SARS-CoV-2, the virus that causes COVID-19, when performing in-person COVID-19 screening of patients, vendors, visitors and staff (all assessment for triage) at building entrances.
