29 CFR 1926.1053(a)(3)(ii):Rungs, cleats, or steps of step stool(s) were less than 8 inches (20 cm) apart or more than 12 inches (31 cm) apart, as measured between center lines of the rungs, cleats, or steps:  (a) On or about 06/17/2020 - Erwin Middle School located at 532 23rd Ave. NW in Center Point, AL, employees were exposed to falls of up to eight feet because the only access from level 2 to the roof of Building C was a job made ladder with rungs that were 19 inches apart
