29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe condition(s) and the regulation(s) applicable to his work environment to control or eliminate any hazard(s) or other exposure to illness or injury:  (a) AAA Roofing and Construction, LLC at 701 Wood Street in Fort Collins, CO 80521: On and before July 16, 2020, an employee was operating a Dewalt table saw, Model DWE745, SN: 202007-DU066586, without the blade guard assembly in place.  Employee's right thumb was amputated when his hand slipped off the push tool he was using to guide foam insulation into the saw blade.  Employee was not properly trained to adjust the blade guard assembly and/or to never use the saw without the guard in place as noted in Dewalt DW745 user manual on page 2.   This condition exposed employee to amputation hazards and laceration of his right hand.

29 CFR 1926.300(b)(1): When power operated tools are designed to accommodate guards, they shall be equipped with such guards when in use:   (a) AAA Roofing and Construction, LLC at 701 Wood Street in Fort Collins, CO 80521: On July 16, 2020, a Dewalt table saw, Model DWE745, SN: 202007-DU066586,  was being operated without the blade guard assembly as required by manufacturer's user manual.    An employee was cutting ISO roofing insulation with Dewalt table saw using a push tool when his hand slipped past the push tool handle, thus making contact with the saw blade.  This condition exposed the employe to amputation hazard of thumb on right hand and deep laceration of the right palm.
