29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest system.  At the jobsite, employees performing roofing activities on the roof were not provided fall protection and exposed to an unprotected fall hazard at a height of over 6 feet to the ground below.   Valentin Rodriguez, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1050693, Citation Number 1, Item number 2, and was affirmed as a final order on May 1, 2015, with respect to a workplace located at 9737 Amaranth Drive, Fort Worth, Texas 76177.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  At the jobsite, employees performing roofing activities were not provided fall protection.  The employer did not provide a training program with hazard recognition and avoidance training where part of the job is to be exposed to fall hazards at a heights over 6 feet to the ground below.   Valentin Rodriguez, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1050693, Citation Number 1, Item number 3, and was affirmed as a final order on May 1, 2015, with respect to a workplace located at 9737 Amaranth Drive, Fort Worth, Texas 76177.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access;    At the jobsite, Employees engaged in installing roofing shingles were exposed to fall hazards while using a ladder that did not extend at least three feet above the roof's landing.  Valentin Rodriguez, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1050693, Citation Number 1, Item number 4, and was affirmed as a final order on May 1, 2015, with respect to a workplace located at 9737 Amaranth Drive, Fort Wort, Texas 76117.
