29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s): a) On or August 5, 2020, and at times prior, Hensel Phelps Construction, Co., as the controlling employer, did not maintain an effective program which provided for frequent and regular inspections of the job site, materials and equipment by a competent person. Employees of Total Welding, Inc. a 2nd tier subcontractor, were exposed to fall hazards greater than 15 feet while conducting stairway landing erection activities in the south stairwell core. Hensel Phelps Construction, Co. did not inspect the subcontractor's fall protection system. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").
