29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems:   On or about August 7, 2020, the employer did not ensure that employees, installing metal decking and insulation, from the roof of a building were in use of fall protection.  This exposed the employees to a fall hazard greater than 30 feet to the concrete ground below.

29 CFR  1926.761(b):  Fall Hazard Training. The employer did not train each employee exposed to a fall hazard in accordance with the requirements of this section. The employer did not institute a training program and ensure employee participation in the program.    On or about August 7, 2020, and at times prior thereto, employees were exposed to fall hazards while installing metal decking and insulation up to 36 feet above the lower concrete ground.  An employee was not wearing fall protection harness.  The employer had not provided fall protection training.  The employer was not familiar with the OSHA requirements for fall protection in steel erection.
