29 CFR 1910.178(a)(4): Modifications or additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: Employee was exposed to fall hazards. The employer did not provide an addition to the powered industrial truck that was approved by the manufacturer. The pallet used by employees that was attached to the powered industrial truck was not approved by manufacturer. Employee was elevated approximately 8 feet while working from a pallet supported by the forks of the powered industrial truck. The pallet the employee was working from was not an attachment approved for use by the powered industrial truck manufacturer.
