29 CFR 1926.501(b)(13): "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.    On or about August 11, 2020, employees were exposed to fall hazards approximately 17 feet 7 inches high while installing solar panels on a residential property.  Fall protection was not being utilized while performing construction operations.      Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1926.1053(b)(4): Ladders were used for purposes other than the purposes for which they were designed:  On or about August 11, 2020, employees used an A-frame ladder in the closed position to access a roof, exposing employees to fall hazards associated with using a ladder for purposes other than designed. 29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladders shall be used at an angle such that the horizontal distance from the top support to the foot of the ladder is approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support).  On or about August 11, 2020, employees used an orange extension ladder at an angle such that the horizontal distance from the top support to the foot of the ladder (approximately 9 feet) is more than one-quarter of the working length of the ladder (approximately 11 feet, 4 inches).  Abatement documentation is required of this item in accordance with 29 CFR 1903.19(d).

29 CFR 1904.29(a): Basic requirement. You must use OSHA 300, 300-A, and 301 forms, or equivalent forms, for recordable injuries and illnesses. The OSHA 300 form is called the Log of Work-Related Injuries and Illnesses, the 300-A is the Summary of Work-Related Injuries and Illnesses, and the OSHA 301 form is called the Injury and Illness Incident Report.    On or about August 11, 2020, the employer failed to use OSHA 300, 300a, or 301 forms at the workplace.   Abatement certification is required for this item in accordance with 29 CFR 1903.19(c).
