29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated, the person certifying that the evaluation has been performed, the date(s) of the hazard assessment, and, which identifies the document as a certification of hazard assessment:  A personal protective equipment (PPE) hazard assessment was not documented.  Personal protective equipment that was applicable included safety glasses, respiratory protection, hearing protection, and gloves, for body work, such as grinding and sanding, and for spray painting.  Abatement documentation is required for this violation.

29 CFR 1910.132(f)(4): The employer did not verify that each affected employee received and understood the required training through a written certification that contained the name of each employee trained, the date of training, and that identified the subject of the certification:  Personal protective equipment (PPE) hazard assessment training and required PPE to be used was not documented.  Personal protective equipment that was applicable included safety glasses, respiratory protection, hearing protection, and gloves, for body work, such as grinding and sanding, and for spray painting.  Abatement documentation is required for this violation.

29 CFR 1910.134(f)(2): Employee(s) using tight-fitting facepiece respirators were not fit tested prior to initial use of the respirator and annually thereafter:  Half mask cartridge respirator was not fit tested before use and annually thereafter for the spray painter.  Solvent based chemical products were used for painting.  Abatement documentation is required for this violation.

29 CFR 1910.1200(e)(1)(i): The employer did not compile a list of the hazardous chemicals known to be present using a product identifier that was referenced on the appropriate safety data sheets:  Although a list did exist in the safety data sheet (SDS) binder it was not comprehensive.  The binder of SDS's was not maintained in a manner where safety data sheets could be readily identified.  Chemical products were used at the business, that included serious hazards, such as serious eye injury and systemic organ affects, as well as flammability.  Abatement documentation is required for this violation.
