29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a. On or about August 12, 2020, and at times prior, at the job site, on a 6:12 pitch roof, three roofing employees were exposed to 10 feet fall hazards from the roof eaves, in that; they were not protected by a fall protection system. The Five Star Building Group, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501 (b)(11), which was contained in OSHA inspection number 1132666, citation number 1, item number 2, and was affirmed as a final order on 04/15/2016, with respect to a workplace located at 37 Fernbrook Drive Saint Johns, FL 32259.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a. On or about August 12, 2020, and at times prior, at the front eave of a roof, four roofing employees were exposed to a 10 feet fall hazard, in that; the roof access ladder rails did not extend at least 3 feet above the upper landing surface of the roof. Five Star Building Group was previously cited for a violation of this occupational safety and health standard, 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 1327412, citation number 1, item number 1a and was affirmed as a final order on September 24, 2018, with respect to a workplace located at 42 Patina Place, Saint Augustine, Florida.
