Location: Oakland Grove Health Care Center, Woonsocket, RI  On or about 8/13/2020, the employer did not provide a medical evaluation to determine each employee's ability to use a respirator before requiring employees to wear respirators. The employer required employees to wear respirators  while providing care to suspected or confirmed positive COVID-19 patients and residents.

29 CFR 1910.134(f)(2): Employee(s) using tight-fitting face piece respirators were not fit tested prior to initial use of the respirator.  a.  Location: Oakland Grove Health Care Center, Woonsocket, RI  On or about 8/13/2020, the employer did not provide a  fit test to all employees who were required to wear N95 respirators. The employer required employees to wear N95 filtering face piece respirators to protect against the SARS-CoV-2 virus while providing care to suspected or confirmed positive COVID-19 patients and residents.

29 CFR 1904.29(b)(3): Each recordable injury or illness on the OSHA 300 Log and 301 Incident Report was not recorded within seven (7) calendar days of receiving information that a recordable injury or illness has occurred.   a. Facility-wide: On or about 5/4/2020 the employer did not record workplace injuries or illnesses on the OSHA Form 301 or equivalent, within the required 7 days for the calendar year 2020. Between 5/4/2020 and 8/10/2020 there were  five (5) incidents of COVID-19 infection or presumed COVID-19 infection, resulting in days away from work. These five incidents include, but are not limited to, workers illness that involved between 4 and 90 days away from work.
