29 CFR 1926.502(d)(15)(ii): Anchorages used for attachment of personal fall arrest equipment were not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached, and not installed under the supervision of a qualified person. a) Jobsite: 2024 N 2nd St, Philadelphia, PA, 3 story building, roof, parapet wall - Employees installing metal siding on the fa�ade of the building from a pump jack scaffold approximately 23 feet above ground level were exposed to fall hazards in that the employer did not have the anchor of the personal fall arrest system used by the employees installed under the supervision of a qualified person. The steel nail in hinged roof anchor fastened on the top of the parapet of the 3 story building was secured with 6 nails when it was designed to be fastened with 32 nails. Observed on or about 8/12/20.
