29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by a guardrail system, safety net system, or personal fall arrest system, or an alternative fall protection measure that meets a provision described in paragraph (b) of this section: a) Lot 23 Log Cabin Road, Milford, DE - On or about August 13, 2020, the employer failed to ensure each employee used fall protection while working on a roof approximately 15 feet above the grade, exposing employees to fall hazards. Avamer Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard (29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1126126, citation number 1, item number 1 and was affirmed as a final order on (10/16/2017), with respect to a workplace located at 202 Ann Avenue, Rehoboth Beach, DE.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder was used to gain access: a) Lot 23 Log Cabin Road, Milford, DE - On or about August 13, 2020, the employer failed to ensure employees used ladders that extended at least 3 feet above the upper landing to access the roof, exposing employees to fall hazards.
