29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use. On east side of the construction site, an employee worked from a platform made of plywood material elevated by a JLG forklift. The work platform was not an approved work platform by the manufacturer exposing the employee to unprotected fall, struck-by and crushed-by hazards.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On east side of the construction site, employees performing framing activities without fall protection were exposed to unprotected fall hazards of approximately 9 feet to the ground below.
