ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation.  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement. IN ADDITION, abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic, or video evidence or abatement or other written records.

Penn Stainless Products, Inc., 102 McQuiston Drive, Jackson Center, PA  16133, production area, grinding table, on or about August 13, 2020 and times prior thereto:  A yellow flexible cord of a cord reel was powering equipment including, but not limited to, a DeWalt DWE43144N 6-inch angle grinder.  Employees were utilizing the angle grinder for various grinding tasks.  The yellow flexible cord had a cut in its outer jacket and inner insulation that exposed the bare copper conductor.    B.)  Penn Stainless Products, Inc., 102 McQuiston Drive, Jackson Center, PA  16133, production area, grinding table, on or about August 13, 2020 and times prior thereto:  A yellow flexible cord of a cord reel was powering equipment including, but not limited to, a DeWalt DWE43144N 6-inch angle grinder.  Employees were utilizing the angle grinder for various grinding tasks.  The yellow flexible cord had a wear mark on the outer jacket that exposed the inner insulated conductor.    NO ABATEMENT CERTIFICATION OR DOCUMENATION REQUIRED
