29 CFR 1910.178(l): Operators were not trained in the safe operation of powered industrial trucks:  At a jobsite located at 206 West 43rd Street, Hays, KS employees engaged in plastering activities were exposed to serious crushing and struck-by injuries. Forklift operator had not completed a forklift training program that met the requirements of OSHA standard 1910.178(l) powered industrial trucks, operator training.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met: (Construction Reference: 1926.59)  At 206 West 43rd Street, Hays, KS, employees engaged in plastering activities were exposed to Alpha base coat, Senershield, and gasoline, which can cause skin and eye irritation, vision loss, loss of consciousness, and cancer in that the employer had not developed and implemented a written comprehensive hazard communication program.
