29 CFR 1926.452(w)(2): Scaffold casters and wheels were not locked with positive wheel and/or wheel and swivel locks, or equivalent means, to prevent movement of the scaffold while the scaffold was used in a stationary manner: At Location: On or about 8/13/2020, the employer exposed an employee to fall hazards of approximately 7.3 feet while painting the façade from a one-tier baker scaffold located in the front entrance of a residential home where the employer did not ensure the scaffold's wheels were in the locked position while being used.

29 CFR 1926.454(a)(1): The employer failed to have each employee who performs work while on a scaffold trained regarding the nature of any electrical hazards, fall hazards and falling object hazards in the work area. At Location: On or about 8/13/2020, the employer did not ensure employees working on mobile scaffolds were trained by a competent person in all hazards associated with scaffolding.
