On or about, August 5, 2020, at 28700 La Caille Drive, in the city of Naples, an employee was exposed to a fall hazard while working from an extension ladder positioned nearly at a 45 degree angle.

Instance 1: On or about, August 5, 2020, at 28700 La Caille Drive, in the city of Naples, an employee was exposed to a fall hazard when the employer did not ensure that a defective extension ladder with a missing rung lock was immediately tagged "Dangerous: Do Not Use" or with similar language.  Instance 2: On or about, August 5, 2020, at 28700 La Caille Drive, in the city of Naples, an employee was exposed to a fall hazard when the employer did not ensure that a defective extension ladder missing the pulley system was immediately tagged "Dangerous: Do Not Use" or with similar language.
