29 CFR 1910.23(b)(8): Ladders were not used only for the purpose for which they were designed: a.) 1160 Chambers Road, Aurora CO 80017: On or about August 6, 2020, and at times prior MK Roofing, LLC employees were exposed to fall hazards while using an extension ladder for a purpose which it had not been designed. An extension ladder had been installed to serve as a fixed ladder to provide access to the roof. Abatement note: Abatement certification and documentation are required for this item (see enclosed Certification of Corrective Action Worksheet).

29 CFR 1910.23(b)(2): Ladder rungs, steps, and cleats were not spaced less than or equal to 14 inches (36 cm) apart, as measured between the centerlines of the rungs, cleats, and steps: a.) 1160 Chambers Road, Aurora CO 80017: On or about August 6, 2020, and at times prior MK Roofing, LLC employees were exposed to fall hazards while using a ladder with a bottom rung 27 inches above the floor in the employee break room. The bottom rung of the ladder was positioned 27" above the floor and MK Roofing employees were using a nearby 5 gallon bucket as a step to access the ladder. 29 CFR 1910.23(d)(2): Employer did not install fixed ladders with a minimum perpendicular distance of 7 inches (18 cm) (for elevator pit ladders, a minimum perpendicular distance of 4.5 inches, 11 cm); from the centerline of the steps or rungs, or grab bars, or both, to the nearest permanent object in back of the ladder; a.) 1160 Chambers Road, Aurora CO 80017: On or about August 6, 2020, and at times prior MK Roofing, LLC employees were exposed to fall hazards while using a ladder with a rung which did not meet the minimum perpendicular distance of 7" from the centerline of the rung. The roof access ladder had the second from the bottom rung obstructed by a wooden bracket which fixes the ladder to the wall. The bracket partially covers the rung and therefore does not meet the minimum perpendicular distance required by the standard. The top of the roof access ladder was level with the roof.
