29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. a) 117 Panda Dr., Shady Springs; On or about June 25, 2020, an employee sustained a work-related injury when the employee fell approximately 8 feet off a ladder while carrying an empty bucket. The injury resulted in an in-patient hospitalization that was not reported to OSHA within 24 hours.
