Unevaluated employees using the forklift to transport items around the maintenance shop in this condition were exposed to struck-by and crushing hazards.

(a) A & J Landscaping, LLC, located in Sylvania, OH: On or about August 18, 2020: The employer failed to ensure an employee operating the Toyota 2500 forklift maintained a position inside the running lines of unit while the unit was in a operational condition. The employee, operating from a position between the mast and outside frame of the vehicle was exposed to crushed-by hazards.
