29 CFR  1926.405(j)(1)(i): Fixtures, lampholders, lamps, rosettes, or receptacles had live parts normally exposed to employee contact:(  a) Employees were exposed to electric shock hazard; there was an exposed live wire tip/terminal screw on the wall mounted light switch inside the doorway to the construction area, the elevator motor room (roof house); the light switch box did not have a cover and there was access to the live wire terminal screws. 29 CFR  1926.405(g)(2)(iv):Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:  a) Employees were exposed to electric shock hazard due to damaged wiring; the metal conduit wire covering was separated exposing the inner wires to sharp edges/damage; on wiring located over the door to the elevator roof house. 29 CFR  1926.451(b)(4): Employer did not ensure that: Each end of a scaffold platform, unless cleated or otherwise restrained by hooks or equivalent means, shall extend over the centerline of its support at least 6 inches (15 cm).  a) Employers suspended scaffold platform extended only 3 inches over its support point due to being obstructed by the roof railing; was used for work on the elevator roof house, elevation 10 stories. Employees were exposed to scaffold collapse hazard. 29 CFR  1926.451(b)(10): Scaffold components manufactured by different manufacturers shall not be intermixed unless the components fit together without force and the scaffold's structural integrity is maintained by the user. Scaffold components manufactured by different manufacturers shall not be modified in order to intermix them unless a competent person determines the resulting scaffold is structurally sound.  a) The employer modified scaffold uses/types; employer attached a suspended scaffold work platform to a pipe scaffold frame standing next to (tied to) the railing at the edge of the roof; elevation 10 stories. The scaffold components were not secured/ used as designed; employees were exposed to scaffold collapse hazard when rebuilding the elevator roof house. 29 CFR  OSH ACT of 1970 Section (5)(a)(1): Section 5(a)(1) of the Occupational Safety and Health Act: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were not protected from the hazard struck by/crush hazard by an elevator which was not locked out.   a) Employees were exposed to struck-by/ caught-by hazard from sudden start-up of the elevator machine (pulleys and cables) due to the elevator not being shut down or guarded during rebuilding of the elevator motor roof house.
